In Balance - Female Focused Weight Loss Intake Form

Are you done with dieting?

Are you ready to start eating?

Are you ready to start feeling better, to have more energy, to sleep better?

Be good to your body. Be good to your self.

My name is Dr Julianne Dunne. I am board certified in obstetrics & gynecology as well as obesity medicine.

I believe that knowledge is power.  The pillar of my weight management program is EDUCATION.  Education about nutrition. Basically what we have all been taught about nutrition over the past several decades is wrong.  The advice we have been given to “Eat less and exercise more” hasn’t worked. We have been eating less and exercising more but this has led to a nation where one in three Americans over obese and 2 in 3 Americans are overweight or obese!

Education is the most important tool for reaching and maintaining a healthy weight. Other tools I offer are medical grade meal replacements.  These are low calorie, carbohydrate-controlled plans that have proven to be the most effective non-surgical treatment for weight loss. Another tool is prescription appetite suppressants, for the appropriate candidate, that help control appetite and cravings.

At your first appointment, we will spend an hour reviewing nutrition and optimal eating regimens. Please fill out the intake form and fax to 914.948.1019 (attention: Dawn) along with recent blood work PRIOR to your appointment. If you have not had blood work in the past few months, we will discuss at your first visit.

I want to review your information prior to your visit so we can spend the entire hour on your education and your healthy eating regimen! I recommend monthly follow-up visits (essential for anyone on weight loss medication) and offer no-charge weekly weigh-in visits.

Start now to avoid all the future complications of excess weight.
The top ten reasons to get on the road to a healthier you.

If you are READY, don’t wait any longer; there is no time like the present!!

Although insurance companies do not currently pay for weight management, they will most often pay for the "co-morbidites" or problems that excess weight can cause. After reviewing your paperwork, we will let you know what codes you can check with your insurance company.
